Health Care Providers

Health Care Practitioners in the Mississauga Halton region have access to clinical tools, resources and services with which to support patients and their families throughout palliative care stages.

The Palliative Care Toolbox is designed for primary care providers as a resource in navigating palliative care in Mississauga Halton and Central West regions, with links to best practice tools tools and local resources.  It is disease agnostic with tools organized according to the 3-step model of best practice proposed by the Gold Standards Framework (GSF): Identify, Assess, and Plan.

In addition, we encourage physicians to review the Academic Detailing: The Palliative Approach to Care in the Mississauga Halton LHIN. Developed by the Mississauga Halton Palliative Care Network, this resource brings together into one document palliative care best practices and local resources.
